What is Vagifem?

Vagifem is a low-dose vaginal oestrogen therapy prescribed to treat symptoms of vaginal atrophy, now more commonly referred to as Genitourinary Syndrome of Menopause (GSM). It comes as a small, bullet-shaped pessary (vaginal tablet) that is inserted into the vagina using a disposable applicator.

Each pessary contains 10 micrograms of oestradiol, a type of oestrogen identical to the one naturally produced by the ovaries. Vagifem delivers this hormone directly to the vaginal tissues, helping to restore moisture, elasticity, and a healthy pH, all of which decline after menopause.[1]

Because the treatment is local rather than systemic, only a very small amount of oestrogen enters the bloodstream. This makes Vagifem an effective and low-risk option for many women.

Why use Vagifem?

After the menopause, oestrogen levels fall significantly. This hormonal drop affects the vulva, vagina, bladder, and lower urinary tract. Common symptoms of GSM include:

  • Vaginal dryness or irritation
  • Discomfort or pain during sex (dyspareunia)
  • Itching or burning sensations
  • Urinary urgency, frequency, or pain
  • Recurrent urinary tract infections (UTIs)
  • Reduced vaginal elasticity, thinning skin, or loss of discharge

These symptoms can significantly affect a woman's quality of life. Many women assume they are ‘just part of getting older’ but they are treatable. Importantly, GSM does not always respond to standard systemic HRT, which is why local oestrogen therapy like Vagifem is considered first-line treatment for persistent urogenital symptoms.

How does Vagifem work?

Each Vagifem pessary delivers a precise low dose of oestradiol directly to the vaginal tissues. This:

  • Rehydrates and thickens the vaginal lining
  • Restores the pH balance, helping maintain healthy microbiota
  • Improves blood flow and tissue integrity
  • Reduces inflammation, helping alleviate urinary and genital symptoms

Most women begin to feel improvement in vaginal moisture and comfort within 2 to 4 weeks. However, full effects, especially for urinary symptoms it may take up to 12 weeks of consistent use.

Unlike moisturisers or lubricants, Vagifem targets the underlying hormonal cause, offering long-term symptom relief when used regularly.
